Case Manager

Outpatient programs provide trauma-informed care aimed at reducing the harmful impacts of substance use. Participants and their counselors work collaboratively to design an integrated, individualized, strengths-based treatment experience geared towards addressing each client’s specific needs and goals. Outpatient programs provide Individual and group therapy, individualized case management, treatment planning, drug and alcohol counseling, crisis intervention, and connection to social support services.

JOB SUMMARY
PRSC Case Managers are responsible for working in conjunction with Subcontracted Community Based Transitional, Sober Living and Employment Services providers (CBPs) to assess participant needs and strengths in support of developing and securing permanent, independent or supportive housing, employment and ancillary resources.
Through an array case management services and providing high quality customer service, Case Managers ensure and monitor that CBPs match participants to appropriate available housing and employment opportunities, tracking outcomes for each participant. Case Managers are also responsible for maintaining up-to-date documentation for each participant.
KEY RESPONSIBILITIES
Case Management Responsibilities: Reviews participant information with CBP and collaboratively coordinates placement of participants directly into post-transitional housing and employment resources. Monitors and tracks CBP services delivery for quality assurance for contracted community programs. Screens project documents and records for QA/IR of reports. Coordinates site visits for both current and prospective Community Based Providers and documents findings on the HealthRIGHT 360 PAR.
Documentation Responsibilities: Responsible for documenting participant status and progress for permanent housing and job placement opportunities. Maintain accurate and up-to-date files for participants including all Probation required documents and ensures receipt of necessary paperwork from CBPs. Tracks outcomes and monitors subcontractor contractual and service provision quality assurance. Identifies and tracks all participants’ outcomes for reporting to County PRCS representatives. Maintains accurate records through entering data/documentation into electronic database for all participants in accordance with guidelines established by HealthRIGHT360 to satisfy internal and external evaluating requirements. Other duties as assigned.
EDUCATION AND KNOWLEDGE, SKILLS AND ABILITIES QUALIFICATIONS:
Education
Required:
- High School diploma or equivalent
-A valid California driver’s license
Desired:
-Bachelor’s Degree in related field
Background Clearance
Required:
-Must complete background check and livescan clearance
- Ability to complete LA County Probation clearance
Knowledge
Required:
-Culturally competent and able to work with a diverse population
-Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications
-Experience working successfully with issues of criminal background, and other potential barriers to economic self sufficiency
