HealthRIGHT 360, a family of programs, operates in 11 counties throughout California. We offer a comprehensive range of medical and behavioral health services. HealthRIGHT 360 serves marginalized communities, providing primary care, mental health support, withdrawal management, residential and outpatient substance use disorder treatment, as well as transitional housing services tailored to individuals involved in the criminal justice system.
Located in a multi-service behavioral health center for Orange County residents, HealthRIGHT 360 provides gender responsive withdrawal management and residential treatment for adult men and women. The program offers a therapeutic milieu to improve the health, safety, and self-sufficiency of people struggling with addiction. The HealthRIGHT 360 Program is a residential substance use disorder (SUD) treatment program that provides 3.1, 3.3, and 3.5 ASAM levels of care for males and females, as well as 3.2 withdrawal management. There are 30 beds allocated for 3.1/3.3/3.5 and 12 beds for 3.2, for a total of 42 beds.
KEY RESPONSIBILITIES
Medication/Floor Duties-LPT/LVN assists with keeping track of and storing medication for clients at the residential facility and helps clients in withdrawal management with taking their medications. Monitors and notifies medical personnel of medication adherence issues or side effects that clients may be experiencing. Helps clients with making outside medical appointments and support ancillary floor staff as needed. LPT/LVN works a block shift schedule and will need to cover evenings and weekends.
Intake Duties-LPT/LVN helps with medical evaluations for new clients coming into the facility and documents findings. Coordinates with other intake staff and other licensed medical providers to ensure that incoming clients are medically stable enough to initiate residential/withdrawal management treatment. If incoming clients require further stabilization, they will work with staff to ensure patients are transferred to appropriate withdrawal management facilities.
Direct Service Duties:
- Assist the nursing supervisor and associate medical director in the day-to-day monitoring, treatment, and maintenance of client medical conditions as qualification and license allows.
- Intake process: Take VS, perform COVID test, as well as TB test, UDS, and UPT if applicable
- Assist with blood draws.
- Facilitate health, wellness, and medical education for staff and clients.
- Develop and assess effectiveness of individualized treatment plans and client progress.
- Complete random and scheduled client UDS tests and document test results.
- Act as liaison for removal of biohazardous waste as well as the proper disposal of discontinued medications and controlled substances.
- Assist with checking in clients when they return from off-site and checking all bags upon entering the facility.
- Perform periodic facility checks to ensure and maintain the safety and security of the facility.
- Conduct random and planned room searches primarily in the withdrawal management units.
Documentation Duties:
- Conduct chart audits and report findings to Medical Management.
- Ensure that MARs (Medication Administration Records) are complete and accurate.
- Maintain documentation in compliance with agency, HIPAA, and DMC ODS standards.
- Comply with all documentation standards and procedures set down by law or funding agency requirements.
- Maintain up-to-date files and timely documentation in the EHR system.
- Monitor and document for medical quality assurance.
- Complete all required paperwork, charting, records maintenance in a timely manner when providing direct services.
- Submit program and contractual reporting and documentation requirement in a timely manner
Administrative Duties:
- Take the lead with transportation requests, client appointments, running Pop Sheet reports, and managing OTC box.
- Communicate with multidisciplinary team in treatment planning and service delivery.
- Establish working relationships with community partners for continuity of care.
- Serve as care coordinator for clients and completion of all coordinated care documentation.
- Arrange work schedule in accordance with agency’s needs.
- Comply with the agency’s policies and procedures.
- Actively participate in agency and team meetings.
- Complete all assigned trainings and Relias trainings in a timely manner.
QUALIFICATIONS
Education, Certification, or Licensure
- State LPT/LVN licensure
- Basic Life Support certification.
Experience
- Experience and commitment to working with diverse communities.
- Experience serving seriously and persistently mentally ill clients with co-occurring challenges.
- At least 3 years of LPT/LVN experience preferred.
- One year work experience in a residential substance use treatment setting or a withdrawal management or withdrawal management setting desired.
