Work Schedule: M-F 8am-12pm (20 hours a week and can work any days M-F, as long as it's 20 hours)

 

HealthRIGHT 360 Behavioral Health Intake is the central access point for all clients seeking behavioral health services in San Francisco. This is a dynamic program with opportunities to interact with clients in need. The Intake Department works closely with the medical clinic to help clients access medical, addiction treatment, behavioral health - including psychiatry, and dental services.

The BH Intake Nurse (IRN) is located in the Intake Department and collaborates to provide triage, care coordination, and clinical evaluation for clients who present to Intake for substance use disorder treatment. The IRN works closely with the Intake Client Access Coordinator (ICAC) to assess clients coming into Intake, especially those seeking detox services, to triage clients which must be seen by the addiction service (MAT provider), triage clients that need immediate escalation of care, i.e., emergency with need for EMS services, and coordinate appointments and care for the clients. 

In collaboration with the Addiction Medicine team, the IRN supports the provision and education regarding Medication Assisted Therapy (MAT) where appropriate. The IRN oversees Drug Medi-Cal documentation compliance; supports other Intake staff; and acts as resource for all other staff in environment of care requirements such as basic safety precautions; and in the containment and cleanup of biohazardous waste, utilizing appropriate PPE and supplies, as needed.

Key Responsibilities

Intake Nursing Responsibilities: 

  • Provides clinical evaluation and triage for clients coming in to access detox and residential substance use services.
  • Provides complex care coordination for all clients accessing healthcare services internal and external to HealthRIGHT.
  • Provides care coordination to obtain information from external healthcare providers, pharmacies, or other resources that may inform the client’s care.
  • Screens and triages hospital referrals for appropriate level of care.
  • Support MAT team to evaluate Intake clients that walk in for services, which may include evaluating and triaging client and/or coordinating evaluation by MAT provider.
  • Review of medications brought in by clients are appropriate. Consults with Addiction provider if any concerns, duplicate medications, inappropriate medications.
  • Refers clients for medical services at the ICC clinic or externally as appropriate. This may include primary care, psychiatry, dental, or addiction services.
  • Coordinating external health information to inform patient care.
  • Share pertinent client information with medical providers as well as behavioral health team as appropriate, this may include medication lists, problem lists, hospital discharge information, etc.

Education and Knowledge, Skills and Abilities

 

Education, Certification, and Experience Required:

  • Active registered nursing license in CA.

  • BLS Certification must be obtained within 30 days of hire. 

  • Experience and interest in working with safety-net populations and in treating substance use disorders.

  • Experience working successfully with issues of mental health, criminal background, and other potential barriers to economic self-sufficiency. 

Desired:

  • Bilingual in Spanish.
