The Withdrawal Management Unit (Detox) at HealthRIGHT 360's Dual Diagnosis treatment facility at 815 Buena Vista (815) is a co-ed unit with 52 beds that provides clients with social model detoxification support for co-occurring behavioral health and Substance Use Disorders (SUD). Clients are supported by a team of licensed nurses, SUD counselors, and behavioral health professionals who provide medication management support, counseling, and education. The On-call (ONC) Withdrawal Management Registered Nurse (WMRN) acts, in tandem with the Medical Director, as the lead in supporting our clients with Medication Assisted Therapies (MAT) as part of their evidence-based treatment plans. Our goal is that our clients transition into their recovery journey with stable physical health to build upon.
The On-Call Registered Nurse carefully assesses and evaluates induction of MAT treatment plans and reports findings to prescribing providers and other stakeholders in the client's care as necessary. They assess and evaluate client vital sign monitoring and observation checks for discontinuation. They act as resource in the medical management of WM clients and may lead a team including but not limited to licensed nurses, licensed psychiatric technicians, and/or medical assistants.
They are responsible for Drug Medi-Cal documentation compliance, such as medical necessity of WM services. They may participate in the ongoing training of WM staff like including quarterly skills-checks. They participate in environment of care interventions such as medication control, storage, and disposal in the milieu; basic safety precautions in compliance with the nurse practice act; and in containment and cleanup of hazardous waste, utilizing appropriate PPE and supplies, as needed.
Key Responsibilities
Intake Nursing Responsibilities:
• Provides clinical evaluation and triage for clients coming in to access detox and residential substance use services.
• Provides complex care coordination for all clients accessing healthcare services internal and external to HealthRIGHT.
• Provides care coordination to obtain information from external healthcare providers, pharmacies, or other resources that may inform the client’s care.
• Screens and triages hospital referrals for appropriate level of care.
• Support MAT team to evaluate Intake clients that walk in for services, which may include evaluating and triaging client and/or coordinating evaluation by MAT provider.
• Review of medications brought in by clients are appropriate. Consults with Addiction provider if any concerns, duplicate medications, inappropriate medications.
• Refers clients for medical services at the ICC clinic or externally as appropriate. This may include primary care, psychiatry, dental, or addiction services.
• Coordinating external health information to inform patient care.
• Share pertinent client information with medical providers as well as behavioral health team as appropriate, this may include medication lists, problem lists, hospital discharge information, etc.
Education and Knowledge, Skills and Abilities
Education, Certification, and Experience Required:
• Active registered nursing license in CA.
• BLS Certification must be obtained within 30 days of hire.
• Experience and interest in working with safety-net populations and in treating substance use disorders.
• Experience working successfully with issues of mental health, criminal background, and other potential barriers to economic self-sufficiency.
Desired:
• Bilingual in Spanish.
