This is a grant funded position through SAMHSA contract (H79SM086855) funded from 09/30/2022- 09/30/2026. This position will be attached to the Pomona Outpatient Behavioral Health site and will work between our Residential and Outpatient programs. The client population consists of pregnant and parenting (perinatal) women, criminal justice involved, mandated and community clientele from different walks of life. The goal of the Certified Community Behavioral Health Center (CCBHC) Program is to integrate mental health and substance use disorder treatment (behavioral health) and physical health allowing clients to receive whole-person, streamlined system of care targeting their primary and behavioral healthcare needs. This position will work as part of a treatment team with a targeted approach to supporting clients transitioning between levels of care to support the highest possible outcomes.

 

The primary responsibilities of this role are focused around supporting clients while transitioning from residential level substance use disorder treatment into outpatient substance use disorder treatment and/or other outpatient ancillary services (mental health services, medication support services, rehabilitation services, case management services, etc), as applicable. Acknowledging and attempting to mitigate identified barriers to successful transitions and participation in services resulting in completion highlight the need for a compassionate, empathetic professional to guide and assist clients during these overwhelming transitions and while complying with treatment requirements. Additionally, leading all employment readiness support services including facilitation of group and individual sessions as the expert in this topic.

Key Responsibilities 

1. Assists the client with participating in the intake process by supporting awareness of the scheduled event and ensure the client’s attendance. 
2. Assess the client’s needs in regard to transition between programs and assist client in reducing barriers to meeting programmatic expectations.
3. Connects the client to appropriate personnel is respective programs in which they will be enrolled (ie Intake Specialists, OP SUD Counselors, Therapists/Assessors, Case Managers, Medication Support Services Staff, etc.).
4. Coordinates communication and service linkage including: assisting with scheduling appointments, communicating with internal providers, and communicating with external professionals (Probation Officers, DCFS workers, etc.).
5. Facilitates group and individual employment readiness supports for enrolled clients. 
5. Assists clients in navigating unfamiliar areas on site in outpatient in which they will receive services.
6. Works collaboratively with other members of the treatment team including the Therapist, Counselor, Medical, and Psychiatrist
7. Meets client where they are to deliver services (in outpatient, residential, or field depending on assignments and client need).
8. Connects the client to benefits, mental health, physical health, employment, probation, DCFS, employment, housing, community resources, outpatient substance use disorder services, and aftercare.

Position Requirements:

 Education Requirements
-Certification as Substance Abuse Counselor from an approved/accredited California agency OR Bachelor’s Degree from an accredited university in a human services field qualifying work as a case manager. 
Experience
-Preferably a minimum of two years’ experience in the human service field and demonstrated expertise in substance abuse treatment, relapse prevention, recovery and/or mental health. 
-Experience working with clients experiencing acute withdrawal from substances and mental health crises.
-Knowledge of and experience with providing trauma informed services.
-Experience delivering evidence-based practices.
Skills:
-Professionalism, punctuality, flexibility and reliability are imperative.
-Excellent verbal, written, and interpersonal skills.
-Bilingual in Spanish (PREFERRED)
