Job Overview

 

Position shift: Sun-Thurs 2pm-1030pm. Candidates must be able to work this schedule.

Prototypes Pomona Recovery Bridge Housing (RBH), a HealthRIGHT 360’s program, provides abstinence-based interim housing for adult and adult perinatal women experiencing homelessness and are concurrently enrolled in outpatient, intensive outpatient, opioid treatment program, or outpatient withdrawal management services. RBH services include peer support, group and house meetings, self-help, case management, life skills development, and other recovery-oriented services. 

The RBH Case Manager I/II/III is responsible for delivering services to a caseload of RBH housing clients and providing individualized substance use disorder case management services. The Case Manager I/II/III works in collaboration with other members of the multidisciplinary team to ensure the best possible treatment outcome for the client and provide treatment in accordance with HealthRIGHT 360’s philosophy, goals, policies, mission, and vision. The Case Manager I/II/III may provide treatment utilizing different modalities including individual and/or group.

Key Responsibilities

Direct Services Responsibilities: 

  • Assists clients with intake, assessment, and orientation to program upon admission or transition in care level.
  • Provides facility coverage for housing program.
  • Monitors client's progress toward achieving treatment plan goals and provides treatment plan input. 
  • Utilizes trauma-informed care while applying motivational interviewing, cognitive behavioral therapy, and evidence-based practices.
  • Facilitates weekly house meetings.
  • Seeks clinical consultation throughout the treatment episode and when encountering new and/or high-risk clinical circumstance. 
  • Works collaboratively with other members of the treatment team including the Case Manager, Clinical Therapist, Medical, and Psychiatrist.
  • Maintains frequent communication with the treatment team and engages in regular consults, clinical team, case conferences, and plan development meetings.
  • Delivers services in outpatient, residential, or field depending on assignments and client need.
  • Responsible for treatment oversight of the clients on their caseload.
  • Assists with client program oversight by managing client business passes, privileges, check-in, and treatment compliance. 
  • Meets expected client care hours (productivity expectation is 25 client care hours per week).

Clinical Responsibilities:

  • Writes and completes all progress notes within 72 hours of service delivery.  
  • Maintains compliance with due dates for all clinical documentation. 
  • Documentation must maintain compliance with agency policy and procedures, HIPAA, 42-CFR p2, and SAPC standards.

Administrative Responsibilities: 

  • Attends and actively participates in assigned supervision.
  • Actively participates in agency and team meetings.
  • Participates in training opportunities and pursues training in Evidence-Based Practices.  
  • Communicates collaboratively with all members of the behavioral health team including: medical, mental health, psychiatry, substance use disorder, and other staff. 
  • Completes all assigned trainings and Relias trainings in a timely manner.
  • Arranges work schedule in accordance with the agency’s needs. 
  • Attends regular staff meetings and other meetings as assigned.
  • Reads emails daily and responds as needed.
  • Transports clients when appropriate.

And perform other duties as assigned.

Education and Knowledge, Skills and Abilities

Education, Certification, and Experience:

  • Bachelor’s degree or SUD/AOD registration with at least a high school diploma or GED.
  • Experience working with clients experiencing acute withdrawal from substances, chronically homeless, and/or reentry populations.
  • Valid California driver’s license.
  • First Aid certified within 30 days of employment.
  • CPR certified within 30 days of employment.

Background Clearance:

  • Must not be on active parole or probation, for a minimum of two years.
  • Ability to obtain and maintain satisfactory background check.
  • Ability to obtain and maintain live scan clearance.
  • Must be fully vaccinated against COVID-19, including booster shot. Medical or religious exemptions not available for this position.
  • Must be able to pass health screening and TB test requirements.

Knowledge:

  • Knowledge of gender-responsive, trauma-informed, and co-occurring treatment.
  • Knowledge of clinical documentation (treatment plans, progress notes, etc.).
  • General understanding of Housing First and Harm Reduction models.

Skills and Abilities:

  • Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data.
  • Strong proficiency with Microsoft Office applications, specifically Word, Outlook, and Internet applications.
  • Excellent verbal, written, and interpersonal skills.
  • Integrity to handle sensitive information in a confidential manner.
  • Excellent organization skills and ability to multitask and juggle multiple priorities. 
  • Ability to work cooperatively and effectively as part of interdisciplinary team and independently assume responsibility.
  • Bilingual preferred.
