Job Overview
This is a union position.
Women age 18 and above with severe substance use disorders are able to enter residential treatment with their children under the age of 10 ½. In addition to treatment of severe substance use disorder, the residential program is able to serve the following specialty populations: pregnant and postpartum women, women with histories of trauma and/or domestic violence, women with co-occurring mental health disorders, women with history of traumatic brain injury, women with open DCFS cases, and women with criminal justice involvement.
The Intake Coordinator II is the first point of care for engaging clients in treatment at Prototypes treatment programs. The Intake Coordinator II is responsible for the initial assessment of candidates to determine need for SUD services, completion of program admission forms, and initial Treatment Plan. The Intake Coordinator II works in conjunction with Supervisor II to provide a warm hand off to the client’s treatment team, assuring new clients are stabilized and urgent needs have been addressed. The Intake Coordinator II will work closely with Counselors, Case Managers, Therapists, and other team members to coordinate and monitor client issues. Often, the Intake Coordinator II works in a group environment and will collaborate with Supervisor II in referring for urgent mental health support. The underlying responsibilities for the Intake Specialist II to the client population include assessment of needs, care planning, education, information, and referral.
Key Responsibilities
Direct Service:
- Provide comprehensive initial assessment of candidates’ needs.
- Ensure clients’ urgent needs are met and appropriate referrals have been made.
- Complete all documentation involved in admissions process
- Begin process of assisting client in applying for Medi-Cal and/or County funding, as applicable.
- Complete Initial Treatment Plan, as applicable.
- Provide a warm hand off to assigned residential treatment staff upon completion of admissions process.
- Provide follow-up to ensure services are obtained.
- Provide crisis intervention as needed within scope of practice.
- Develop collaborative relationships with community partners.
- Communicate effectively with inter-disciplinary team and participate in team meetings to review cases.
- Document relevant and required information regarding each client.
- Maintain and observe all 42 CFR pt. 2 and HIPAA regulations that apply.
Clinical Documentation:
- Write and complete all progress notes within 72 hours of service delivery.
- Maintain compliance with due dates for all clinical documentation.
- Write client progress letters and court reports and obtains final approval from the supervisor.
- Documentation must maintain in compliance with agency policy and procedures, HIPAA, 42 CFR pt. 2, and SAPC standards.
- Complete all assigned URC’s within a timely matter.
Education and Knowledge, Skills and Abilities
Education, Certification, and Experience:
- Board of Behavioral Sciences (BBS) registration or licensure (ACSW, MSW, ASW, LCSW, LMFT, AMFT, APCC, LPCC) required.
- First Aid Certified within 30 days of employment.
- CPR Certified within 30 days of employment.
- Preferably a minimum of two years’ experience in the human service field and demonstrated expertise in substance abuse treatment, relapse prevention, and recovery.
Background Clearance:
- Must not be on active parole or probation.
- Ability to obtain and maintain satisfactory background check.
- Ability to obtain and maintain live scan clearance.
- Must be able to pass health screening and TB test.
