The Integrated Care Center Clinic is a federally qualified health centers that provide primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We provide care to those most in need including those who are suffering from homelessness, mental illness, substance use, mental illness, and chronic medical conditions. We provide team based care in an integrated care setting. Looking to make a difference in the lives of homeless and at-risk communities in San Francisco? Interested in working with a multi-disciplinary team in a non-judgmental environment to meet the needs of patients in an integrated primary care, behavioral, and oral healthcare? At our Integrated Care Center, primary care providers work with a team of nurses, medical assistants, case managers, mental health providers, dentists, and other specialists to deliver care and improve health outcomes for underserved populations. The Registered Nurse is an essential clinical team member who provides high quality, client-centered care to our clients. They provide support throughout the course of a clients clinical journey, working in a team-based model with providers and other medical staff to assess clients, provide individualized care plans and ensure follow up. An important part of the nursing role is to foster a supportive and collaborative environment for both patients and staff.
Works on Patient Care team to provide nursing support including the following:
- Provides patient care understanding orders, health education, and advanced patient care support.
- Ensures primary care team support of patient encounters, medical case management and quality improvement.
- Provides complex care management for patients with multiple chronic medical illnesses and behavioral health concerns.
- Supports patient care team to ensure efficient clinic flow.
- Coordinate care with other care teams and specialists.
Leadership Responsibilities: Responsible for leadership within the quality improvement infrastructure of the medical program. Participates in matters related to performance and quality improvement, planning, protocols and goal setting. Comfortable in changing systems, champions change, and leads change processes.
Performance and Quality Improvement Responsibilities: Assists in goal setting and performance improvement planning across the organization. Writes, reviews, and makes recommendations for policies and procedures for nursing or organizational processes. Active participant in Quality Improvement activities and may act as change champion or lead. In conjunction with Clinic and/or Medical Director is responsible for working with external stakeholders on audits, reviews, or surveys and supports internal efforts as appropriate.
Training Responsibilities: Participates in for training medical assistants, nursing students, and nursing volunteers to improve the skills of back office staff. Orients new nurse volunteers, clinical and support staff to appropriate policies, procedures and protocols of the medical clinic. Supports staff training in quality improvement functions related to their specific position; e.g., basic customer service and patient interaction skills; clinical documentation processes; incident reporting; confidentiality and privacy. Provides training on clinical and quality improvement issues, as needed and appropriate.
Program Development and Project Management Responsibilities: Develops program areas that support nursing care, the patient care team, and patient continuity of care. Participates in grant/funding program development activities as appropriate.
Nursing Responsibilities: Manages “same day” patients with primary care team. Provides advice to primary care team walk-in patients and patients calling into clinic. Provides nursing support to the patient care team and managing clinic flow. Provides appropriate patient care visits and manages patient panel under standing orders. Manages prescription refills and medication modification for patients under standing orders. Provides complex care management for patients with multiple chronic medical illnesses and behavioral health concerns for patient panel. Works closely with staff collecting and reporting contractual data as necessary. Functions as a nurse within the scope of California BRN licensure and credentialing and privileging of the agency.
Education and Knowledge, Skills and Abilities Education and Experience Required:
- ASN, BSN, MSN or equivalent degree.
- Graduate of an accredited School of Nursing, Active California RN license.
- BLS Certification (must obtain upon hire).
- Background Clearance Required: Must not be on active parole or probation.
Knowledge Required:
- Culturally competent and able to work with a diverse population.
- Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications.
- Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency.
- Preferred: Experience with electronic medical records. Comfort working on teams and with safety net populations. Experience in managing volunteers. Spanish as additional language preferred.
