A Day in the Life
The RN Case Manager provides trauma-informed and medically related case management services for the residents of the Kosair for Kids Complex Care Center. Working under the direction of the Director of Case Management and in collaboration with physicians and the interdisciplinary team, you will support residents and their responsible parties to achieve the highest quality of life and access necessary resources during admission, transitions, and discharge.
How This Position Contributes to Our Mission
- Plan, coordinate, and facilitate resident admissions, transfers, and discharges in collaboration with referral sources, physicians, and other stakeholders to ensure seamless continuity of care.
- Collaborate with residents, responsible parties, and the interdisciplinary team to develop and implement individualized comprehensive care plans addressing medical and psychosocial needs.
- Ensure residents and families are equipped with resources, training, and support to transition effectively during admissions and discharges.
- Maintain professional relationships with referral sources and external providers to enhance care coordination.
- Advocate for residents and their families to achieve optimal emotional, psychosocial, and physical well-being.
Schedule
- Hourly, Monday-Friday, 9:00 AM - 5:00 PM
Skills and Qualifications
- Required: Associate Degree in Nursing (ADN); active and unencumbered Kentucky Registered Nurse (RN) license; current CPR/Basic Life Support certification.
- Preferred: Bachelor of Science in Nursing (BSN); knowledge of long-term skilled nursing care regulations.
- Strong interpersonal, communication, and analytical skills.
- Empathy and emotional stability to provide support to residents and families in challenging situations.
- Proficiency with Microsoft Office Suite and related software.
