Social Worker Position Summary:

As a member of the interdisciplinary team, provides social work services to patients and families as assigned. Services include assessment, emotional support, assistance with financial and other social service needs, support and guidance regarding end-of-life decisions, legal affairs, and family assistance in planning for caregiving.

Social Worker Position Qualifications:

• Must at least possess a Licensed Social Worker designation in Kentucky with a baccalaureate degree in social work or social welfare program accredited by the Council on Social Work Education. Master’s prepared social workers possessing a Certified Social Worker or Licensed Clinical Social Worker designation with the state of Kentucky are also well qualified.
• Minimum one-year social work experience in health care.
• Technology proficiency, including use of Microsoft Suite, Electronic Medical Record, and videoconferencing.
• Excellent communicator, with ability to communicate with a variety of internal and external stakeholders.
• Must be adaptable; able to work with clients of differing social, economic, political, or religious lifestyles or beliefs.
• Strong problem-solving skills and ability to work independently.
