Are you passionate about improving healthcare access and investing in provider networks? HPSM is seeking a Provider Grants Program Manager to develop, implement, and evaluate capacity funding programs that support our provider network. Reporting to the Director of Provider Services, this role plays a vital part in advancing HPSM’s Network Investment Strategy and delivering value to our members and community.

Key Responsibilities
- Lead Network Investment Initiatives:
Plan, execute, and evaluate programs that align with HPSM’s strategy for strengthening provider networks through grants and capacity funding. - Grant and Funding Program Management:
Oversee all aspects of grants and funding programs, including provider applications, funding recommendations, contract execution, and vendor management. - Build Provider Relationships:
Act as a liaison with awardees, providers, and community partners to support funding opportunities, practice improvements, and resource sharing. - Evaluate and Improve Programs:
Use quality improvement methodologies to design program evaluations, analyze data, gather feedback, and recommend enhancements. - Monitor and Communicate Progress:
Develop dashboards, track performance metrics, and share updates and outcomes with stakeholders to ensure alignment with organizational goals. - Research and Innovate:
Stay informed on industry trends, best practices, and regulatory requirements to guide program improvements and investment recommendations. - Documentation and Reporting:
Maintain program documentation, including regulatory reports, policies, and procedures, while ensuring compliance with standards. - Support Cross-Functional Collaboration:
Facilitate decision-making, coordinate work plans, and collaborate with internal and external stakeholders to achieve program goals
Requirements

Education and experience
- Bachelor’s degree in healthcare, economics, public policy, or a related field (Master’s preferred).
- At least three (3) years of program management experience; one (1) year in quality/process improvement is preferred.
- Experience in healthcare or Medicaid managed care settings is strongly preferred.
Knowledge & Skills:
- Expertise in project and program management methodologies.
- Familiarity with provider reimbursement, healthcare economics, and grant management best practices.
- Proficiency in data analysis, quality improvement tools, and regulatory compliance.
