Summary

Plan, coordinate, develop, and facilitate individualized plans for adults with DD and other challenges in a community-based setting in conjunction with the residential provider. Promote the principles of self-determination and community integration of persons served. Advocate in the participant’s best interest in accordance with their client rights specified in the Ohio Revised Code.

Essential Job Functions

  • Ensure appropriate service delivery and active treatment for 30 or more participants in conjunction with residential providers, FCBDD, family members, and other involved individuals and/or agencies.
  • Schedule and facilitate pre- and post-evaluation meetings. Ensure that orientations, skills assessments, and follow-up surveys are completed on a timely basis and in a manner understandable to the person served.
  • Ensure the development and implementation of individualized, person-centered plans and services designed to maximize each individual’s independence and membership in the community.
  • Maintain updated and, a minimum of quarterly, accurate case records on the progress of persons served.
  • Provide timely, ongoing contact and problem-solving as the primary liaison with the referring agency, family members, and other involved agencies.
  • Serve as a member of the Interdisciplinary Team.
  • Ensure that all staff is properly trained on all Behavior Support Plans/guidelines and the implementation is in accordance with all governing rules and regulations as required.
  • Facilitate groups/individual sessions for participants including anger management, social skills training, and communication skills development as needed and/or as requested.
  • Review daily documentation to ensure accuracy and to monitor accountability and compliance with rules, licensures, and standards.
  • Maintain a safe workplace and comply with all state and federal safety and health regulations as well as company safety policy and procedures.

Ancillary Job Functions

  • Participate as an active member of the Case Records Committee and attend all required meetings.
  • Participate in all staff meetings, Services Coordinator meetings, and all mandatory in-services.
  • Assist in the coordination/training for staff including Individual Plan development, incident report writing, and behavior intervention training such as C.I.T.I.E.S.
  • Provide transportation as needed for appointments or meetings, etc., as requested.
  • Assist with personal care including transferring participants as needed.
  • Perform other related duties as assigned.

Education and Experience – An equivalent combination of education, training, and experience will be considered.

  • Bachelor’s degree in psychology, social work, rehabilitation, counseling, special education, or related field; or an associate degree in MH/MR/CD and completion of a bachelor’s degree.
  • Two years of experience working with individuals with disabilities. Licensed Professional Counselor or Social Worker preferred.
  • Must have current, valid OH driver’s license and personal car insurance.
  • Must maintain First Aid, CPR, and Medication Administration certifications and other required training.

Knowledge, Skills, and Abilities which may be representative, but not all-inclusive of those commonly associated with this position.

  • Knowledge of ODMRDD, FCBMRDD, Adult Services, CARF, and Goodwill Columbus rules and regulations.
  • Knowledge of current concepts, techniques, and trends in case management, Day Services, and the DD field.
  • Demonstrated ability with person-centered planning and behavior support required.
  • Ability to provide therapeutic intervention required.
  • Ability to represent Goodwill Columbus to participant families and others in a positive, realistic, and professional manner.
  • Ability to exercise strong independent judgment in determining appropriate techniques, approaches, and communications with participants, family members/guardians, and outside agencies to maximize satisfaction for persons served.
  • Clear verbal communication skills and ability to give and follow written and oral instructions.
  • Ability to effectively organize and prioritize work as well as concentrate on multiple tasks simultaneously.
  • Ability to effectively solve problems, use good judgment, and think quickly and rationally in difficult or stressful situations.
  • Knowledge of Microsoft Office programs and ability to use a personal computer, calculator, and related office equipment.

Work Environment – Environmental or atmospheric conditions commonly associated with the performance of the functions of this job.

  • Exposed to moderate noise levels.
  • May be exposed to potentially hazardous bodily fluids.
  • May be exposed to potentially dangerous physical situations when participants exhibit assaultive or inappropriate behaviors.

  • Regularly required to talk, hear, use close vision, and be able to focus.
  • Frequently required to sit, stand, and bend at the knee and waist and walk.
  • Required to reach and hold on to items at chest level or reach above the shoulder.
  • May be required to assist with lifting and transferring individuals weighing more than 100 pounds and to participate in crisis intervention situations that require the ability to bend, stoop, kneel, and hold participants in a therapeutic manner as trained.
  • Must have finger dexterity and hand-eye coordination to work on computers, telephones, and related office equipment.
  • Regular attendance is a necessary & essential function.
