Summary

 

Support and assist individuals with developmental disabilities or other barriers to increase person-centered, individualized integration and vocational exploration in the community.

 

Essential Job Functions

  • Assist individuals with disabilities in the community in discovering their likes, activities, dislikes, goals, and volunteer/work experiences (Discovery).
  • Design and implement skill-building activities in naturally occurring settings (conflict resolution, interpersonal skills, transportation safety, and communication, etc.).
  • Build and maintain effective, strategic partnerships at community sites that foster independence and provide a variety of integration and skill-building opportunities through volunteer and work experiences.
  • Complete required documentation (assessments, daily documentation, and attendance), recommend changes, and communicate progress to individual teams.
  • Utilize and analyze assessments to ensure person-centered planning services are implemented.
  • Provide coaching and mentoring in the community to individuals to achieve full integration.
  • Monitor the health and safety practices of participants, complying with all company, state, and federal regulations.
  • Increase social networks of individuals with developmental disabilities through the facilitation of social relationships.
  • Develop and maintain personal schedules.
  • Participate in behavior support practices that are in line with individual behavior support plans and encourage a culture of understanding, empathy, and support. Provide verbal and/or physical intervention as needed to keep the individual and other workers safe.
  • Provide transportation to individuals to/from Adult Day Services activities throughout the day in a safe, efficient manner.

Ancillary Job Functions

  • Assist with personal care and med admin as needed.
  • Coordinate transportation as assigned.
  • Advocate for individuals as they build independence through community exposure.
  • Attend team meetings to discuss participant behavior, health, and goals.
  • Balance finances daily for participant petty cash as assigned.
  • Other duties, as assigned.

 

Education and Experience – An equivalent combination of education, training, and experience will be considered.

  • Must have a two-year college or technical degree in education, social and human services, therapy, or a health-related field with 1 to 3 years experience working with individuals with developmental disabilities, OR must have a high school diploma or equivalent with 3 to 5 years experience working with individuals with developmental disabilities.
  • College-level coursework in developmental disabilities, geriatrics, or DD is helpful.
  • Must have First Aid, CPR/AED, medication administration, crisis intervention, and other required training.
  • Experience in providing active treatment and personal care to individuals preferred.
  • Must have current, valid OH driver’s license, personal car insurance, and pass a DOT Physical.

  • Exposed to potentially dangerous physical situations when participants exhibit assaultive or inappropriate behaviors.
  • Community settings with potentially frequent changes in settings.
  • Required to drive company vehicles or vans, at times in high-traffic conditions.
  • Exposure to potentially hazardous bodily fluids.
  • Must load and unload participants from vans, sometimes in inclement weather.

  • Ability to effectively organize work as well as concentrate on multiple tasks simultaneously.
  • Ability to prioritize the needs of individuals while out in the community.

  • Knowledge of the city and ability to network within the community.
  • Ability to utilize technological resources in the community.
  • Ability to portray customer service skills while building community partnerships.

  • Must be able to stand, walk, run/jog, reach, and bend for periods while assisting participants.
  • Assists with pushing wheelchairs for participants as needed.
  • May be required to operate mechanical equipment related to activities or using wheelchair lifts.
  • Must be able to lift up to 100 pounds with assistance.
  • Must be able to frequently assist an adult with positioning or sitting.
  • Regular attendance is a necessary and essential job function.
  • Must be able to provide and/or assist with personal care duties.
  • Must be able to move; bend to strap/fasten wheelchairs in vans.
