RN Case Manager

- Summary
Nursing, caring for the medical needs and well-being of our clients on-site.
Essential Job Functions
- Provide exceptional care to our clients with experience in administering medications prescribed, medical treatment and checking vital signs, performing any treatments or testing prescribed by a physician and responding to any medical emergencies, setting up MARS.
- Contact the participant’s home staff for changes in medical status or to communicate on issues to improve the care or safety of participants attending the program.
- Document activity in patient records and obtain updated physician orders on a regular basis. Ensure orders are on file for any incident reports, and intakes.
- Maintain accurate medical charts for each participant. Notify staff of any significant changes in diagnosis or medication.
Ancillary Job Functions
- Respond to any page or call a nurse; evaluate and treat the patient as needed.
- Participate, as requested, in committees that need a nursing presence such as QA or incident review.
- Perform other related duties as assigned.
Education and Experience – An equivalent combination of education, training and experience will be considered.
- Licensed in Ohio as an RN or LPN.
- Experience as an RN or LPN and caring for developmentally disabled individuals.
- Must maintain CPR/AED and First Aid certifications.
Knowledge, Skills and Abilities which may be representative, but not all-inclusive of those commonly associated with this position.
- Clear verbal communication skills and ability to give and follow written and oral instructions.
- Ability to effectively organize and prioritize work as well as concentrate on multiple tasks simultaneously.
- Ability to work independently and make responsible decisions.
- Ability to use good judgment and think quickly and rationally in difficult or stressful situations.
Work Environment – Environmental or atmospheric conditions commonly associated with the performance of the functions of this job.
- Exposed to moderate noise levels.
- Exposed to potentially hazardous bodily fluids.

- Must be able to stand, walk, push, grasp, bend and reach for extended periods.
- Must have finger dexterity and hand-eye coordination to work on computers and related office equipment.
