What you will do:

 

We are looking for a Supervisor of Care Management who can help shape our vision and support our mission. Here is what the position will look like.

  • Responsible for the direct supervision and leadership of care management or coordinator staff. This will include but is not limited to; maintaining performance & compliance standards, adhering to contract requirements, ensuring policies and procedures are met and conducting QA audits.  
  • Serves as the primary resource for staff; maintains and assigns appropriate caseload/workload and prioritizes responsibilities across team. Ensures all deadlines are met, holds regular 1x1s for individual staff and conducts regular team meetings as per department standards.
  • Provides coaching and feedback, performance evaluations, managing staff concerns, and disciplinary actions as required.  Tracks, monitors and reports individual and team performance and is responsible for setting clear expectations with staff.
  • Reviews all documentation requiring supervisory oversight and signature, including, but not limited to timesheets, leave requests, and expense reports. Is knowledgeable and follows applicable labor laws to manage staff.
  • Responsible for collaborating with HR/CM recruiting team to interview and hire staff.  Supports job-shadowing schedules, onboarding training needs and identifies ongoing training elements. Will ensure all required COA and CM training elements are completed.
  • Leads communication efforts through ongoing information sharing (individual/group) presentations, staff meetings, written communication and reference materials that support internal knowledge or member information.
  • May represent Colorado Access at external meetings as requested by manager.
  • Supports required documentation and reporting for individuals, team, program, LOB related to care management.

 

 

What you will bring:

 

Education:  Bachelor’s degree in Business, Healthcare Management, and/or Human or Behavioral Health Science field; or an equivalent combination of education and experience, with relevant equivalent experience is preferred.

 

Experience: Three years’ experience required in related field. Two years supervisory experience in healthcare environment preferred. 

 

Knowledge, Skills, and Abilities: Knowledge of healthcare system and strong understanding of Medicare and Colorado Medicaid or payer organizations.  Must have excellent written and verbal communication skills, customer service skills, ability to work independently. Demonstrates support for the company’s mission, vision and values.  Must have strong time management skills, strong interpersonal skills and a strong working knowledge of databases, clinical documentation systems, word processing, and spreadsheet programs.  May be required to manage multiple priorities and projects with tight deadlines.  Position requires flexibility to use complex processes and procedures to facilitate quality outcomes and/or resolutions while working with other team members with varied skills and educational background.

 

Licenses/Certifications: A valid driver's license and proof of current auto insurance will be required for any position requiring driving.

Pay, Perks and Benefits at Colorado Access:

 

The compensation for this position is $69,300.00 -  $90,600.00 annually. The pay rate/salary is commensurate with experience.

Where you will work:

 

This position will be a hybrid model work environment, a blend of ‘In-Office’ and ‘Remote.’
