Concordia Visiting Nurses is seeking ** MULTIPLE** full-time or part-time, Registered Nurses to serve our patients in the South Hills Area. Graduate nurses are welcome to apply!

POSITION SUMMARY

The Registered Nurse is a licensed health care professional who administers skilled nursing care to patients requiring part-time, intermittent visits. She/he provides complex care and supervises the patient/family and other members of the nursing team. Nursing services are provided to clients in accordance with the physician’s plan of care. Nurses who serve as primary nurses utilize a case manager approach in assessing, implementing, coordinating and evaluating client care.

QUALIFICATIONS

  • Registered professional nurse with a current valid Pennsylvania license
  • Minimum of two [2] years of current experience as a professional nurse in a medical/surgical work area and one [1] year of home health experience preferred
  • Skilled in critical thinking, communication, negotiation and collaboration
  • Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state
  • DUTIES AND RESPONSIBILITIES

Clinical Skills

  • Provides skilled nursing care that includes activities such as teaching, treatments, including IV therapy, venipunctures, complex wound care and health promotion practices
  • Carries out preventive, rehabilitative and restorative nursing procedures appropriate for the patient’s care and safety.
  • Assesses signs and symptoms in conjunction with reporting reactions to treatments, including drugs, and changes in patient’s physical or emotional condition to the physician
  • Develops the plan of care cooperatively with the physician, patient and family with revisions made based on patient condition
  • Instructs and counsels the patient and family members regarding the care requirements of the patient in the home
  • Assigns and instructs the home health aide, in accordance with the physician’s plan of care
  • Supervises and evaluates the care given by the home health aide to his/her primary patients every 14 days
  • Participates in the evaluation of home health aides who are providing services to his/her primary patients
  • Obtains a medical history, particularly as it relates to the present condition, from the patient and/or family member at the start of care.
  • Conducts a brief physical examination and nursing assessment of the patient at each skilled visit
  • Evaluates the patient, family member[s] and home situation to determine the health teaching required at each skilled visit
  • Evaluates the patient’s condition and home situation to determine what assistance will be available from family members in caring for the patient
  • Coordinates total patient care, serving as the primary nurse for designated patients and evaluating the effectiveness and the appropriateness of care
  • Interprets nursing and other services of the agency to the patient and family as part of planning for care
  • Serves as the primary agency coordinator of care when nursing and other services are provided
  • Plans with the patient, family and other team members for the patient’s discharge from service
  • Completes all paper requirements of the agency within timeframes required by the conditions of participation
  • Meets with Clinical Supervisor or Case Manager weekly for an update of patients under his/her care
  • Documentation
  • Completes the nursing evaluation and start of care submitting to the office within 24 -48 hours or by 5 PM on Monday if the admission is performed over the weekend.
  • Completes required documentation for ongoing services on the day of the visit and submits it to the office daily. All documentation must be submitted by 9:00 am on the day following the last day of the month
  • Updates the medication sheets on an ongoing basis
  • Writes Add Orders from the physician as they are received
  • Completes all required recertifications within last 5 days of the certification period
  • Documents all verbal orders obtained from the physician: To include: Physician name, date, time, visit frequency, duration of visits and signature at Start of care, recertification, resumptions of care, etc.
  • Addresses all areas of the skilled nurses note at each visit
  • Completes the transfer summary as soon as he/she is aware of the transfer and submits to the office
  • Completes the discharge oasis and discharge order and summary to the office within 48 hours of the discharge
  • Updates clinical information on primary assigned patients at each visit
  • If patient complains of pain, completes a comprehensive pain assessment
  • Completes insurance authorization requests and provides the information to the person responsible for insurance authorizations in a timely manner
  • Documents all communications on the MD/care coordination form
