Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. Concordia Visiting Nurses is seeking a dynamic, energetic Home Health Branch Manager Registered Nurse RN  or  Physical Therapist PT for a full-time position, which will report into our Administrative office near Butler, PA.

 

The Home Health Branch Manager  is responsible for organizing and directing the branch office’s on-going functions. The Home Health Branch Manager  oversees the day-to-day operations of the office. The duties of the position include the supervision and coordination of staff to ensure provision of quality patient care and effective team performance to meet agency goals in accordance with agency policies, mission, and vision.

QUALIFICATIONS for the Home Health Branch Manager:

  • Registered Nurse (RN) or Physical Therapist (PT) with a current Pennsylvania license
  • Seven [7] years’ experience in health care reflective of progressive responsibility including management in home health care
  • Interpersonal communication and organizational skills with evidence of leadership roles and responsibilities
  • Thorough knowledge of electronic medical records
  • Knowledge of home health care policies, federal and state regulations, and third-party payers
  • Reliable, independent means of transportation for use in business activities.  Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state.

 

Duties and Responsibilities for the Home Health Branch Manager:

  • Pursues opportunities to grow/promote home & community-based services
  • Collaborates with partnering hospitals to ensure communication and specific hospital best practices and standards are met. Will attend hospital meetings and will assist to correct any issues or concerns that arise.
  • Provides opportunities to expand the services of the branch and agency
  • Provides leadership in promoting and maintaining standards for the delivery of high-quality care by all members of the clinical team
  • Applies problem solving abilities to specific organization/clinical complex patients
  • Works with the management team to assure regulatory and certification compliance at local, state and federal levels
  • Provides guidance to ensure that correct documentation is completed by all clinical staff and meets all regulatory requirements
  • Assists with the development of administrative and operating policies and procedures, reviewing existing policies and procedures: at least annually and revising accordingly
  • Assures that a sound, confidential, secure electronic medical records is maintained, including administrative and clinical records
  • Assures that each patient receives quality-based appropriate service by monitoring staff assignment, clinical review, and coordination of care by all disciplines
  • Hires, evaluates, and terminates clinical and administrative employees to maintain a qualified staff in sufficient numbers to serve patients’ needs
  • Maintains confidentiality of patient and staff information
  • Collaborates with the Medical Directors to ensure sound clinical practices are followed
  • Works collaboratively with the liaison and marketing team to ensure clinical operations are aligned with marketing
  • Organizes monthly staff meetings ensuring space for the meeting is secured and content of the meeting is corporately driven as well as office specific
  • Works collaboratively and communicates needs with centralized services at the corporate office: intake, scheduling, coding, and telehealth services
  • Reviews complaints from patients, families, and physicians
  • Responsible for quality, financial and office metrics including turnover rates for the office
