Hospice Social Worker
Good Samaritan Hospice, a mission of Concordia Lutheran Ministries, provides compassionate home care, facility based, and inpatient hospice services to our patients and their families. Our home care coverage area spreads throughout several counties in Western Pennsylvania with inpatient locations in Wexford, Cabot, and Beaver. The Good Samaritan Hospice team works together to provide relief through pain control and symptom management, while offering emotional, social and spiritual support for the patient and family. POSITION SUMMARY
The Hospice Social Worker performs the psychosocial assessment; provides care to hospice patients in accordance with the individualized plan of care developed by the patient and/or family, physician and hospice team; and coordinates discharge planning (when appropriate) for the hospice patient/family in both inpatient and home care settings.
QUALIFICATIONS
- Master’s degree in Social Work required
- Solid computer skills, including Word
- Excellent organizational skills with attention to detail
- Outstanding communication skills (both written and verbal)
RESPONSIBILITIES
- Works with the Interdisciplinary Group (IDG) to coordinate and update the plan of care for hospice patients/ families
- Provides education to patients/families on coping skills and anticipatory grief as appropriate
- Provides counseling, answers questions, offers emotional support, and follows through on identified problems for patient and family members
- Facilitates conversations related to advanced directives, powers of attorney, and funeral planning
- Coordinates community resources as needed (i.e., referral sources, financial resource identification)
- Performs initial and ongoing psychosocial and bereavement evaluations and provides appropriate support for caregivers at the time of death
- Provides assessment, counseling, crisis intervention, patient/family education, discharge planning, patient/family advocacy, and bereavement
- Attends and participates in IDG meetings
- Participates in required on-call responsibilities
- Demonstrates an understanding of the QAPI process, hospice policies and procedures, and participates in projects when requested
