Job description

Concordia Visiting Nurses is seeking a dynamic professional to fill the dual role of Clinical Supervisor/Palliative Care Managerat our South Hills office location. This is a full-time position with benefits.The Clinical Supervisor is a RN who monitors and evaluates options and services needed to meet a patient’s health needs in cooperation with field staff. Strong communication and resource management skills are a must to promote quality, cost effective outcomes.The Palliative Care Manager is responsible for the management and coordination of the palliative care services. The Manager ensures continuity, cost effectiveness and quality of operations. Facilitates communication and education, demonstrates leadership to those providing palliative care and acts as a liaison between Hospice and Concordia Visiting Nurses. Directly supervises all palliative care nurses, and members of the Interdisciplinary Team as assigned/appropriate.
