Nurse Manager / Wound Nurse (LPN/RN)

Managerial on call weekend rotation
Position Summary:
The Nurse Manager is an LPN/RN who is responsible for care plans, QA process, and supervision of nursing staff. Wound Care experience preferred
Qualifications:
· Dedicated to providing high quality care.
· Graduate of an approved school of nursing
· Current Pennsylvania licensure as a Registered Nurse or Practical Nurse.
· Demonstration of leadership ability.
· Displays caring attitude towards residents.
· Works well with people.
· Show enthusiasm and motivation.
· Demonstrates the ability to verbally communicate resident status/changes in condition.
· Demonstrates the ability to complete written communication required by State/Federal Regulatory
Guidelines and Facility Policies.
Essential Duties and Responsibilities
1. Determines Resident needs so that nursing care can be planned, implemented and evaluated.
2. Make daily rounds to assure that nursing service personnel are performing required duties, and to assure that appropriate nursing care is being rendered to meet the needs of the residents.
3. Delegates Nursing Care/other assignments to appropriate team member based on Resident needs/staff abilities.
4. Communicates with Physicians regarding Resident’s care and condition. May round with Physician as time permits.
5. Assures all documentation of care is complete and accurate.
6. Works closely with families to facilitate problem solving. Maintains excellent communications to make them aware of changes in status, hospital admission, planned diagnostic testing and falls.
7. Participate in admission review of residents in relation to nursing needs and the capability of the department meeting those needs.
8. Participate in Discharge Planning, developing and implementing Resident Care Plans, Nursing Assessments, etc., to meet the total needs of the residents.
9. Make daily rounds to assure that nursing service personnel are performing required duties, and to assure that appropriate nursing care is being rendered to meet the needs of the residents.
10. Assists with direct resident care, assists unit nurses, and functions as charge nurse in his/her absence when necessary.
11. Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards and Concordia policies and procedures. Maintains confidentiality of resident and employee records and information. Has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to a supervisor/manager or to the Corporate Compliance Officer.
