THE POSITION
Concordia Visiting Nurses is seeking a Registered Nurse for a full time position which will cover our Bethel Park Service Area. A wonderful coverage area to accommodate your work life balance. Graduate nurses are welcome to apply!
QUALIFICATIONS
- Graduate of state-approved school with course in pharmacology
- Current license in the State of Pennsylvania
- Minimum of two [2] years experience in patient care
- Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state.

DUTIES AND RESPONSIBILITIES
Clinical Skills
A. Provides skilled nursing care that includes activities such as teaching, treatments, including IV therapy, venipunctures, complex wound care and health promotion practices
B. Carries out preventive, rehabilitative and restorative nursing procedures appropriate for the patient’s care and safety.
C. Assesses signs and symptoms in conjunction with reporting reactions to treatments, including drugs, and changes in patient’s physical or emotional condition to the physician
D. Develops the plan of care cooperatively with the physician, patient and family with revisions made based on patient condition
E. Instructs and counsels the patient and family members regarding the care requirements of the patient in the home
F. Assigns and instructs the home health aide, in accordance with the physician’s plan of care
G. Supervises and evaluates the care given by the home health aide to his/her primary patients every 14 days
H. Participates in the evaluation of home health aides who are providing services to his/her primary patients
I. Obtains a medical history, particularly as it relates to the present condition, from the patient and/or family member at the start of care.
J. Conducts a brief physical examination and nursing assessment of the patient at each skilled visit
K. Evaluates the patient, family member[s] and home situation to determine the health teaching required at each skilled visit
L. Evaluates the patient’s condition and home situation to determine what assistance will be available from family members in caring for the patient
M. Coordinates total patient care, serving as the primary nurse for designated patients and evaluating the effectiveness and the appropriateness of care
N. Interprets nursing and other services of the agency to the patient and family as part of planning for care
O. Serves as the primary agency coordinator of care when nursing and other services are provided
P. Plans with the patient, family and other team members for the patient’s discharge from service
Q. Completes all paper requirements of the agency within timeframes required by the conditions of participation
R. Meets with Clinical Supervisor or Case Manager weekly for an update of patients under his/her care
Documentation
A. Completes the nursing evaluation and start of care submitting to the office within 24 -48 hours or by 5 PM on Monday if the admission is performed over the weekend.
B. Completes required documentation for ongoing services on the day of the visit and submits it to the office daily. All documentation must be submitted by 9:00 am on the day following the last day of the month
C. Updates the medication sheets on an ongoing basis
D. Writes Add Orders from the physician as they are received
E. Completes all required recertification's within last 5 days of the certification period
F. Documents all verbal orders obtained from the physician: To include: Physician name, date, time, visit frequency, duration of visits and signature at Start of care, recertification, resumptions of care, etc.
G. Addresses all areas of the skilled nurses note at each visit
H. Completes the transfer summary as soon as he/she is aware of the transfer and submits to the office
I. Completes the discharge oasis and discharge order and summary to the office within 48 hours of the discharge
J. Updates clinical information on primary assigned patients at each visit
K. If patient complains of pain, completes a comprehensive pain assessment
L. Completes insurance authorization requests and provides the information to the person responsible for insurance authorizations in a timely manner
M. Documents all communications on the MD/care coordination form
Communication
A. Calls the physician for a verbal order to continue visits at start of care, resumption of care and re-certifications
B. Interacts with field staff, patients, families, physicians, other health disciplines, agencies, insurance companies, hospitals, labs, employers, government agencies and representatives as necessary
Professional Standards
A. Protects patient privacy and confidentiality by being discreet and professional in sharing
information on a need to know basis
B. Adheres to all policies set forth in the Nurse Practice Act and maintains a professional standard of conduct, projecting a positive image of the agency
C. Complies with infection control and safety policies and procedures
D. Serves as a role model for all staff
E. Adheres to all policies and practices of the agency to maintain established Standards of Care
Professional Development
A. Attends a minimum of one continuing education seminar annually related to clinical and/or regulatory issues
B. Seeks clinical collaboration in new and/or unfamiliar care areas
C. Participates in staff meetings, care conferences, education training and the OBQI program as appropriate.

Pay: $32.00 - $36.00 per hour
