Summary:

The family Coordinator for the School-based Early Support Program plays a pivotal role in providing comprehensive support and intervention services to children and families within the school setting. This position requires a compassionate and organized individual with a strong understanding of child development, family dynamics, and community resources. The Family Coordinator will work closely with school staff, families, and community partners to assess needs, develop individualized plans, and coordinate services to ensure the well-being and academic success of children.

Responsibilities:

  • Conduct thorough assessments of children and families to identify strengths, needs, and goals. 
  • Collaborate with school personnel and other stakeholders to gather relevant information and develop comprehensive support plans. 
  • Utilize evidence-based practices and assessment tools to inform decision-making and goal-setting. 
  • Coordinate and facilitate access to a range of support services, including educational, mental health, medical, and social services. 
  • Act as a liaison between the school, families, and external service providers. Monitor service delivery and progress towards goals. 
  • Support, advocate and educate for the needs and rights of children and families within the school and larger community. .
  • Documentations and reporting
  • Performs other related duties as assigned.
  • THIS IS A FIELD POSITION. Not in office or remote.

 

 

Qualifications:

  • Bachelor’s Degree in Social Work, Psychology or Social Services.  
  • At least 2 years’ experience in related field. 
  • Good interpersonal skills, ability to work with diverse social, economic, and cultural groups.  
  • Proficiency in Microsoft Office.
