Cornerstone Beach 41st - Outreach Coordinator (Referral Navigator)

In this role you will:
The Referral Navigator is responsible for supporting participants in identifying and creating a network of resources; including but not limited to coordinating and leveraging resources within the agency and external providers. The Referral Navigator will work with the local Cure Violence provider and other neighborhood resources to create a network of support for the Case Management Enhancement Program and throughout the community. Initial outreach will be made to participants to identify a connection with the Social Worker and the Cornerstone.
- Coordinate with Clinical Coordinator/ Social Worker in supporting participants in identifying and creating a network of resources.
- Create opportunities to engage youth 14-24 years old to resources and various programing. Create bi-weekly programs/events for youth to build and established continued relationships.
- Conduct community outreach, build and establish relationships with community-based organizations CBOs; including working with community-based organizations (CBO’s).
- Coordinate group development incentives and activities, including meals, recreation, trips, cultural events, and other opportunities for connection and growth.
- Work with the local Cure Violence provider and other neighborhood resources to create a network of support for the participants.
- Engage participants regularly, encouraging participation and connection to the program.
- Update social medias of upcoming and past events for the Case Management Enhancement Program.
- Performs other related duties as assigned.
Position Type: Full Time
Hours: 11am to 7pm
What qualifications do you need:
- Bachelor’s Degree in Social Work or related field.
- 1 years’ experience in related field.
- Excellent communication and interpersonal skills and ability to work with diverse social, economic, and cultural groups.
- Ability to plan and carry out assignments independently.
- Ability to prioritize, adhere to timelines and multi-task.
- Proficiency in Microsoft Office.
Qualifications Preferred:

- Bilingual in one or more of the following: Spanish and or Creole.
