Quality Improvement - Health Risk Assessment Coordinator 195-1034

JOB SUMMARY:

The Health Risk Assessment Coordinator is responsible for ensuring completion of a health risk survey for all newly enrolled Medicare and Individual Marketplace members. Responsibilities include identifying health risks scores, generating reports and distributing data to the responsible departments. This role supports the QI team member outreach activities including education for Medicare and Commercial members.  This role spends 100% of time on quality program activities.

KEY RESPONSIBILITIES:

• Conduct and organize data pertaining the health risk assessments survey, including regular audits, reporting and distribution of HRA risk scores for newly enrolled Medicare and Individual Marketplace members to internal departments.
• Identifying potential health risks for Medicare and the Individual Marketplace membership.
• Outreach to selected members through various channels.  Including reminders, assisting with HRA questions and survey completion.  
• Supports Star Rating and HEDIS gaps in care measures, specifically related to quality improvement projects for CMS, Accreditation & State requirements.
• Perform other duties as assigned.

QUALIFICATIONS:

• Highly organized and attentive to detail.
• Demonstrated ability to relate and interact effectively with all types of individuals.
• Possess Strong oral and written communication skills.
• Capable of managing multiple tasks at one time.
• Proficient in Microsoft applications.
• Successful completion of Health Care Sanctions background check.

EDUCATION/EXPERIENCE:

• High School diploma or equivalent.
• Experience working with elderly or geriatrics preferred.
• Three years previous customer service experience in managed healthcare, preferred.
• Knowledge of Medicare programs, preferred.
