Clinic Nurse Manager
The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.
The Colorado Coalition for the Homeless seeks a Clinic Nurse Manager who under the Associate Director of Nursing (DON), this position will manage nursing services and functioning of Stout Street Health Center (SSHC). This will be done in accordance with current federal, state, and local standards, guidelines, and regulations that govern CCH to ensure that the highest degree of quality care is managed at all times.
This position is both administrative and skilled nursing care to the patients at SSHC. This position is located at 2130 Stout Street, Denver, CO 80205 and reports to the Associate Director of Nursing.
Qualified candidates are eligible for the NURSE Corps Loan Repayment Program which offers payment of 60% of unpaid nursing education debt in exchange for a two-year commitment at a NURSE Corps approved site with an option to extend to a third year for an additional 25% of the original balance.
NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions
- Directly supervises health center-based Registered Nurses and Nurse Coordinators. This includes the various personnel actions related to hiring, orientation, performance appraisals, counseling, staff disciplinary issues, and staff scheduling. Supervisory activity may include auditing charts and running reports to ensure quality care.
- Responsible for competencies of all clinical nursing staff members, ensures clinical nursing quality of care, and ensures all nursing staff members are practicing within their perspective scope, at highest level, in accordance with the State of Colorado.
- Holds weekly 1:1 supervision with direct reports, including permanent, temporary, agency, or volunteer staff who are filling these roles. Provides feedback, coaching, mentorship, training, support, competency evaluations, and performance management feedback.
- Maintains a high standard of care on the team by offering or assigning relevant trainings, providing clear expectations of best practices, policies, and procedures, evaluating staff with quality assurance metrics, such as auditing direct client care and documentation practices, monitoring environment of care, and supporting remediation or counseling as necessary.
- Consistently adheres, maintains, improves, and updates the standard work for the Nurse Manager position.
- Monitors RN FTE, and schedules team according to established ratios. Ensures the communication and scheduling of nursing staff for each relevant clinical position’s daily needs.
- Represents SSHC around nursing services and supportive health service activity operations, staff engagement, patient care, and workflow operations at appropriate health center leadership meetings.
- Participates in and leads quality improvement projects for SSHC and nursing department.
- Ensure collaboration of care and consistency in all integrated health suite teams, upholding the ongoing part of our best practices policy.
- Communicates, organizes, and co-leads discipline meetings on a regular basis in conjunction with other RN Leadership. Facilitates, leads, or participates in other leadership, discipline, or inter-discipline team meetings as appropriate or assigned.
- Champions and leads quality improvement initiatives for the team. Shares knowledge with and coaches other RN team or inter-disciplinary members on quality improvement & LEAN methodology.
- Helps to create and coach a culture within the health suite teams which fosters and reflects collaboration, mutual respect, and trust.
- Helps to ensure that communication among the integrated care team is regular and routine by facilitating and/or delegating a schedule of regular suite team huddles as well as write care plans for certain individual patients receiving care with CCH.
- Ensures pertinent information is shared across disciplines and within the other health care suites, and other CCH resources as appropriate.
- Acts as a leader to share information and problem solve with other CCH Leadership for the purpose of continuous improvement to patient care and staff needs.
- Brings personnel issues to the attention of appropriate supervisors for resolution.
- Provides skilled nursing care according to accepted community standards of care in order to preserve and/or enhance the patient’s highest level of functioning.
- Available for workday or evening clinic as scheduled or appropriate.
- Maintains EHR core competencies as evidenced by being one of the designated NextGen super users and trainers for nursing services staff members.
- Uses telephone template to record communications regarding a patient.
- Records all patient visits into Nextgen, helping to ensure that PMH, PSH, FH, social history, and allergies are recorded.
- Uses inbox to manage tasks.
- Records vital signs into Nextgen.
- Completes intake note and submits a superbill in Nextgen to bill for RN services.
- Enters immunizations into Nextgen.
- Ability to enter current patient medications into the medication template.
- Performs other duties as assigned.

Qualifications Summary
- Registered Nurse, currently licensed in the State of Colorado.
- Preferred Bachelor Science of Nursing degree (BSN).
- Two (2) years experience as a supervisor in a community health center, hospital, long-term care facility, or other related health care facility preferred.
- Two (2) years experience as an RN. Ambulatory, public or community health, home health nursing, or urgent care experience preferred.
- Certification in Basic Life Support (BLS)-CPR.
