Director of Social Services

The Commons in Lincoln is seeking a Director of Social Services for our Skilled Nursing Rehab community. As the Director of Social Services, you will have the unique opportunity to work with the team across campus while supporting residents' transitions of care. Your role offers both an opportunity to make an impact on our residents' lives along with their families, resulting in an incredibly satisfying and professionally rewarding experience.

Responsibilities:

• Spearhead/coordinate admission meetings with residents and families from admission through to discharge
• Promotes psycho/social wellbeing through individualized resident centered care planning
• Actively participates in resident assessments
• Maintains great relationships with community providers to plan for residents' next level of care
• Supports Advance Directives process

Requirements:

• Current BSW license required, MSW or LICSW preferred
• Minimum 1 to 2 years of experience in a skilled nursing facility preferred
