Job Summary:

With a high degree of independence provides comprehensive case management for the People we Serve maximize our clients potential, maintain the highest quality of life and ability to function within the community.

Description

Assesses treatment needs and develops care plans with mutually established  goals for the people we serve Monitors and assesses client progress by conducting routine evaluations. Collects and analyzes treatment/results data, maintaining accurate client records.

Communicates and coordinates treatment plans to the client and the client’s guardian/family. Addresses client needs, serving as an advocate when problems and issues arise.

Knowledge: Theoretical or practical understanding of a subject acquired and increases through experience. Skills:

Proficiencies developed through training, practice, or experience. Abilities: The level and quality of being able to do something improved with training and experience

 

  • Ability to handle complex case management issues and problems, Ability to identify and implement creative solutions and solve complex, challenging and changing situations.
  • Articulate and confident; must possess poise and show discretion and the highest degree of confidentiality and ethics.
  • Excellent verbal and written communication skills, including the ability to understand nuance and adapt communication style based on the audience and situation.
  • Excellent organizational and ability to plan project and complete satisfactorily
  • Excellent decision-making and conflict resolution skills

Minimum Job Qualifications:

 

Education:

Bachelor’s degree in related field or commensurate experiences

 

Experience:

4 Years of Case Management Experience
