Job Summary:

The Manager Residential Operations - Case Manager is a key member of the AbleLight team that delivers person centered care and services to those we support to fulfill our purpose for existing “We believe the world shines brighter when people with developmental disabilities achieve their full potential.”  Their focus is to facilitate the planning, development, and implementation of person-centered plans and ensure access to needed medical, social, educational, and other services and lead residential program(s)

Essential Duties:

  • In partnership with operational leaders, human resources, quality, and nursing: welcome, onboard, and train and mentor new colleagues on the team and identify additional training and development needs for all staff as part of Quarterly Connections.
  • Demonstrates positive leadership behaviors leading the way through individual behavior
  • In collaboration with operational leaders and colleagues, ensure standard AbleLight operating procedures and any related state and local regulations and expectations are implemented consistently across the homes in your area of accountability.
  • Be the resource for any DSP /Lead DSP colleague, person we support, or parent & guardian in the home if local home efforts to resolve a topic are not successful.
  • Bring recommendations forward related to quality and process improvements for our employees and those we serve to enhance standard operating.
  • Complete all individual training and development required by local, state, federal and AbleLight policies and procedures.
  • Maintains a strong and collaborative working partnership with all colleagues, leadership, PWS, and parents & guardians with a focus on budget and metrics to positively influence the program, grow residential services and to achieve our balanced scorecard results.
  • Facilitates the planning, development, implementation, measuring progress and reassessment of person-centered plans by completing assessments, evaluations, regular review of data, assimilating information into the individualized support plan or other applicable plans and reports, monitors progress towards meeting outcomes. 
  • Assisting people with Intellectual Disabilities in gaining access to needed medical, social, educational, and other services.
  • Providing or coordinating the provision of at least the following direct case management activities to adults with Intellectual Disabilities
    • Needs assessment, including a written comprehensive assessment of the individual’s strengths, weaknesses, needs, and desires.
    • Case planning, including facilitation and participation in the development, review, and implementation of the Plan of Care within appropriate time frames.
    • Service arrangement, including assisting the individual (or family) in identifying necessary services and supports and in linking the individual with needed services/resources as identified in the Plan of Care
  • Social support, including assisting the individual/family in expanding or establishing a social support network through linkage with appropriate persons, support groups, and/or agencies.
  • Reassessment/follow-up, including evaluating progress toward accomplishing objectives specified in the Plan at quarterly intervals, contacting agencies/programs providing services to the individual to evaluate progress/effectiveness of services provided, documenting objectives/ progress, and participating in any revisions needed in the Plan of Care
  • Attending and participating in interdisciplinary and interagency meetings as required
  • Completing necessary documentation as required for maintenance of the individual’s case management/service coordination record as well as for accountability and billing purposes.

  • Ability to handle complex case management issues and problems, Ability to identify and implement creative solutions and solve complex, challenging and changing situations.
  • Articulate and confident; must possess poise and show discretion and the highest degree of confidentiality and ethics.
  • Excellent verbal and written communication skills, including the ability to understand nuance and adapt communication style based on the audience and situation.
  • Excellent organizational ability to plan project and complete satisfactorily.
  • Excellent decision-making and conflict resolution skills

 

Minimum Job Qualifications

  • Education: Bachelor's degree in related field. (May vary by state requirements)
  • Experience: Four Years of Case Management Experience (may vary by state) may vary by states requirements). Two to three years of previous leadership experience.
