Transition Coordinator

Job Summary

Transition Coordinators are assigned at each level of care within the Community Bridges, Inc. (CBI) systems of care network to ensure that patients/patients have an individualized discharge plan. All follow up appointments for ongoing care and services will be scheduled within 7 days of discharge. Transition Coordinators are responsible for coordinating with Utilization Management on all authorizations and continued authorizations. Transition Coordinators will adhere to all policies and procedures. The Transition Coordinator will complete an initial assessment, and treatment plan, as well as engage the individuals and their support system. The Transition Coordinator will provide therapeutic services to individuals and their support systems to include counseling, group work, discharge planning and authorization coordination.

 

Skills/Requirements

 

  • Highschool diploma or GED and either: o 18 credit hours of post-high school education in a field related to behavioral health and two years of full-time behavioral health work experience; or o four years of full-time behavioral health work experience. • Has an associate’s degree and at least two years of full-time behavioral health   work experience; or
  • Has a bachelor’s degree in a field other than behavioral health and at least one year of full-time behavioral health work experience; or
  • Has a master’s degree or bachelor’s degree in a field related to behavioral health.
  • Full-time health care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical situations required.
  • Experience with administrative or operational processes in a health care setting preferred.
